Gathering information about you and what you are looking for in a therapist is paramount. Your goals for therapy will usually be discussed during your first session and throughout therapy. It is important that you feel like you are able to express your needs for therapy.
During your first visit, the the necessary medical and disclosure forms will be gone over. In order to save time during your first session, please read and complete the following forms and notifications. Please print each of these documents, read them, fill out pertinent information, and bring them to your first visit.
